Able to live in a shared or structured housing environment

Our Intake Process

Submit an Intake Form: Complete our online form or contact our team to start the process.

Initial Review: Our staff reviews your information to ensure program eligibility.

Interview / Assessment: We schedule a short meeting or phone call to understand your needs, goals, and challenges.

Program Matching: Based on your eligibility, we connect you with the appropriate housing coordinator.

Move-In & Support Planning: Once accepted, we work with you to coordinate a move-in date.
